Casino Listings is up for best casino affiliate and best overall affiliate I think.

That is where I voted. I think I voted for them in a couple of categories, but I can't remember which ones: I do think it was more than two, but I am not sure. My memory is terrible. lol

You just put in your name, whatever company you work for I guess, and email.
